It means it failed to create, try to use a transcode profile which copies the files from source.
When trying to add logo's or custom profile transcodes sometimes ffmpeg skips some files.

Xtreamui has lots of issues when creating 24/7 channels. What I did is. I've built a script from scratch that allows me to even use opening ending and transition videos. I made this with web interface to create the channels from local or remote files using php, html and ffmpeg. And a php script that points to the link of the channel and prints the content of the segments similar to xtreamui ts playing function. So I put that link in xtreamui with a one minute delay and works perfectly. By doing it this way I have advantages, cause the channel can be played with or without opening and/or transition or to play videos in the order of creation or randomly and to limit to one connection per ip if I want to play the links directly without using xtreamui, and even adding videos to the channel is possible if new seasons comes out, etc. So I recommend you to do something like that.

As I said I've made it with php, html, javascript and ffmpeg. The programming behind this was a lot of work. Months of it, so I won't give the script for free. But If you're good on programming you could achieve what I did in a week maybe.
