I try add stream and getting error.

you need change permisson of folder /upload to 777

check the config file setting.
you should use localhost az server address and just change password

and I found one more error.I click Add Line left menu page created automatic bulk user in database

Via phpmyadmin create a transcode profile, it's failing because you dont have one.
